The Social Support Officer within the Residential Team, is responsible for providing essential support focusing specifically on providing a care service aimed at fostering independent living.

The SSO is to refer to the below-existing areas of responsibility which include providing an effective care service; applying for client-centred support; promoting social, personal, and life skills development; effecting crisis intervention and mentoring; collaboration with the multidisciplinary team; client advocacy; and documentation and reporting. This comprehensive set of roles and responsibilities outlines the collaborative efforts required from the Social Support Officer in supporting the services and initiatives towards the provision of impactful services and programs to the neediest in the community whilst referring them to internal and external support services to ensure a holistic service ensuring a positive supportive network. 1. Care Service: - Support service users in admitting into the service by performing an assessment and admission process. - Help the service user develop their personal resilience and mental well-being by cooperating with the psychotherapeutic services team in the referral process and in the completion of the psychosocial assessment, which will guide the goals of the care plan. - Develop and implement a care plan, in liaison with external professionals, that focuses on fostering independence and enhancing the overall well-being of the assigned service users. - Supporting the service user care plan by performing any required intervention as directed by the Residential Coordinator (RO), which may include, but is not limited to, performing urine and drug testing. - Support the service-user well-being by maintaining a smooth running of the residential facilities; by ensuring service-users abide by their accommodation agreement, which includes but is not limited to abiding by wake and sleep times, ensuring service-user duties are completed, service users beds are being made, bathrooms are being cleaned, common shared spaces are clean, and the entire premises (including staff stations) remain clean, orderly and without clutter. 2. Client-Centered Support: - Support the clients in following their assigned care plan, in liaison with external professionals, to support the service users in transitioning towards independent living and developing a strong social support system. - Provide individualized support to the service users based on the care plan designed to assist in the journey towards independent living and the creation of a positive supportive network. - Actively involve the service users in the provision of the service itself and consider their opinion, promoting service-user involvement. - Conduct regular assessments to identify specific needs and challenges, adjusting care plans accordingly. - Ensure that service users are all safe, supervised, and accounted for during the service provision. - Promote values of individualization, non-judgemental attitudes, and a non-discriminatory approach, even in the most basic communication promoting the importance of these values and cultural humility. 3. Social, Personal, and Life Skills Development: - Assist in facilitating mentoring sessions to enhance clients' life skills, such as personal hygiene, cleaning, budgeting, cooking, time management, and other essential skills for independent living, as per the needs of the clients. - Encourage and support clients in actively participating in any skill-building activities and services being provided by our services, or externally, as a means of also supporting their journey towards strengthening their social support. - Encourage and empower service users to take ownership of their journey toward independence. - Help the service users develop their social and personal skills through conflict resolution, management skills, group work, community meetings, recreational activities, and mentoring. - Crisis Intervention and Mentoring: - Provide immediate support and crisis intervention when required. - Provide mentoring to service users, addressing day-to-day emotional and social challenges. - Provide emotional, social, and spiritual support by listening and empathizing, setting boundaries to behavior, and acting as a good role model. - Follow protocols, policies, and procedures in place during the service provision ensuring day-to-day activities are being met. - Collaboration with Multidisciplinary Team: - Work collaboratively with the broader Services Team, including Social Workers, Coordinators, Psychotherapists, Counsellors, Social Support Officers, and the external Professionals involved in the specific cases assigned. - Participate in regular team meetings and case conferences to ensure holistic and coordinated care. 4. Client Advocacy: - Advocate for clients' needs within the organization and the immediate Coordinators in the Services Team as well as services offered by other entities. - Ensure clients have access to necessary services and opportunities both internally and externally. - Refer where appropriate, internally to services within the Services Team to ensure a holistic service is being provided in the life-cycle of the service users (prevention, outreach, residencies, community & aftercare). 5. Documentation and Reporting: - Maintain accurate and up-to-date records of client interactions and progress through the Client Management System during the service provision. - Prepare regular reports detailing the outcomes of the individual client's achievements. 6. Specific Areas of Responsibility: - Every Social Support Officer will be given a specific area of responsibility by the Residential Coordinator
